Health Ministries Network of Minnesota is a nonprofit, 501(c)3, interfaith organization committed to encouraging, supporting and developing whole-person ministries throughout Minnesota. We welcome people from adjoining states to be part of this network as affiliate members

Care Teams

Care Team® is a group of 12 or more laypeople from a congregation who offer support to individuals and families affected by chronic or terminal illnesses or disabilities. Care Team® members work in groups, sharing service activities so that no team member is solely responsible for the level of care that may be required.

Care Team® members provide a range of services, including:

• Emotional, physical, and non-proselytizing spiritual support
• Limited transportation - to worship, shop, or clinic
• Friendly visits and companionship
• And, for caregivers of ill or disabled loved ones, respite opportunities that lift the burden of care.
